We provide provide face-to-face, video chat interactions through telehealth monitoring with patients to monitor and assist in adherence and reducing readmissions, while improving clinical patient outcomes with our Remote Care Coordinators (RCC). Harnessing this technology allows our coordinators to develop personalized patient care with established protocols through its ability to visually record patient vitals, assess patient needs for medication refills, food consumption, transportation to medical appointments and record social interactions.

    Continuing our care following discharge from the hospital, Home Care for the 21st Century Albuquerque improves patient care with Remote Care Coordinators and Telehealth Monitoring technology. Using the most advanced technology in the industry, our customizable technology kit comes complete with a pre-loaded tablet, blood pressure monitor, pulse oximeter, weight scale, and optional devices used to track patient vitals (i.e., thermometer, glucometer adapter, and spirometer flutter).

    Our tablets feature easily accessible educational videos, specific to the patient’s needs. Video calling, video triage, alerts for medication, access to patient data via secured (HIPAA compliant) Care Portal and has many other customizable features. We provide services that reduce false-positive clinical escalations - thereby reducing the gap in chronic care, avoidable readmissions and healthcare expenditures.

    Weekend Tuck-In Program

    Home Care for the 21st Century Albuquerque's Weekend Tuck-in Program is unique to the industry. Patient care continues regardless of the time, or day of the week. On weekends, home health care providers often have reduced patient care loads. That's why we ensure that our Remote Care Coordinators (RCC’s) pay special attention to the patients’ needs for the upcoming weekends to ensure you are provided the care you need.

    Each patient is contacted prior to the weekend to confirm that they have all they need at home to transition through the weekend without incident. Our Remote Care Coordinators from our Telehealth Monitoring service will personally check with the patient about where they are with supplies, medications, food, any necessary transportation they may need and an understanding of what they need to do. The results of this program are higher satisfaction scores, fewer readmissions and improved quality, at a lower cost.

    How It Works

    1. CONNECTION

    Each patient is assigned a remote care coordinator (RCC) who will monitor medical devices and data, track and adjust the execution of the care plan and coach and guide the patient.

    2. DEPLOYMENT

    A simple, intuitive tele-health kit will be personalized for each patient. This technology captures health data based upon clinically defined thresholds.

    3. OBSERVE

    Obtain clinically meaningful data to enhance and adjust care while developing an appreciation for the patient’s needs, lifestyle choices and motivations.

    4. INTERVENE
